Form IEPF-5 Documents Checklist for Share and Dividend Claims
Use this Form IEPF-5 document checklist to organise identity, bank, demat, folio and entitlement records before filing an IEPF claim.

Separate claim data from supporting documents
Form IEPF-5 captures the claimant, company, folio or demat account, shares and unpaid amounts being claimed. Supporting documents prove identity, payment instructions, demat credit details and entitlement. A strong file keeps the values in the form consistent with the evidence submitted to the company.
The exact checklist depends on whether the claimant is the registered holder, a joint holder, nominee, legal heir, successor, administrator or authorised representative. Obtain the company's current checklist before sending original or notarised papers.

Core records for a straightforward holder claim
The official instruction kit and company checklist should be read together. Common records include the submitted IEPF-5 acknowledgement, claimant identity and address proof, cancelled cheque or other bank proof, and a Client Master List where shares are claimed. Old certificates, dividend warrants and company correspondence support folio identification.

Additional documents for nominee or legal-heir cases
When the registered holder has died, the company must verify who is entitled to claim. The file may include a death certificate, nomination record, succession certificate, probate, letters of administration, legal-heir certificate, no-objection documents or indemnities depending on the facts and current rules.
The 2024 amendment to the IEPF rules recognises specified legal-heir documentation with supporting indemnity and no-objection conditions in relevant cases. Do not assume that one document is sufficient for every family situation or value of holding.

Final consistency check before submission
Compare the spelling and order of names, folio, company CIN, share quantity, dividend years, bank account holder and demat account holder. Check that scans are readable, certifications are current and the postal packet matches the online claim.
Keep a complete duplicate set and a document index. Record the SRN, dispatch date, tracking number and date on which the company receives the file. This makes a later clarification or resubmission much easier to answer.

Is a Client Master List required for an IEPF share claim?
A current demat-account record is generally needed for credit of approved shares. Follow the IEPF instruction kit and the company's checklist for the accepted format and attestation.
Do I need original share certificates?
Requirements depend on the claim and company records. If certificates exist, include their details and follow the company's instructions before sending any original document.
Are legal-heir documents the same for every IEPF claim?
No. The required succession evidence depends on nomination, the claimant category, applicable law, value and whether entitlement is disputed.
